Butler Memorial Hospital (BMH) is the centerpiece of Butler Health System
(BHS). We are pleased to ensure that our patients enjoy a premium care
experience at our Butler, PA hospital. My BMH Health is a service designed
to streamline patient access to their medical records. This page answers
questions commonly asked about My BMH Health.
I do not have an email address, how do I get one?
There are many free email services available to use. Any email service
is compatible with My BMH Health. If you are looking to create a free
email account, some of the more established email providers are:
How can I update my email address in My BMH Health?
You can change your email by selecting preferences at the bottom of the
screen after you have logged onto My BMH Health. You can update preferences
on the right hand side of the screen and follow the directions. Hit submit,
then go back to home and select Profile this changes the information on
the hospital side.

What information can I see in My BMH Health?
You can see information from your medical record such as laboratory and
radiology test results, medications, allergies, immunizations, medical
history, current health issues and due dates for preventive care. You
may also print a Health Summary for your records.
Is there a fee to use My BMH Health?
No. My BMH Health is a free service offered to all of our patients with
How is the My BMH Health site secured?
We take great care to ensure your health information is kept private and
secure. Access to information is controlled through personal IDs and passwords.
My BMH Health uses the latest encryption technology. All My BMH Health
messaging is done while you are securely logged on to our website.
Some devices do not contain the necessary encryption certificates needed
to access the site securely. You can test your browser compatibility

Can I also see my family member's health records?
For a child under 13, a parent or legal guardian can log into their personal
My BMH Health account and then view information in regard to their child
once a proxy form is filled out and signed accordingly. These Proxy forms
are obtained at Medical Records at Butler Memorial Hospital. At the 13th birthday, the minor will then automatically be dropped from the parent
or legal guardians My BMH Health. Records needed for any minor ages 13
to 18 can be obtained at Butler Memorial Hospital Release of Information
Monday through Friday 8 am to 430 pm with the appropriate identification.
It is also possible to request access to another adult's health record
if you help manage that adult’s medical care. This is done by a
Proxy Release of Information which is a Form available at the Medical
Records Department with appropriate identification. For more information,

What if I need more information than is available through My BMH Health?
If you need additional information other than what is available through
My BMH Health, please contact the Medical Records Department at 724.284.4530.
How can I signup?
If you know your Medical Record Number and you have provided your email
address when you last registered, you can
click here and click on the Enroll button.
If you do not know your Medical Record Number then you must fill out a
Release For Access form. Once the form has been received and processed, you should receive an
email detailing next steps. Processing normally takes up to five business
days after our office receives the completed form.
What version of Internet Explorer is needed to use My BMH Health?
Internet Explorer 7 (IE7) and above with web version 8 is needed to use
the Health Portal.
What browser can be used with the health portal?
My BMH Health is compatible with most web browsers. The health portal
has been tested with Safari, Chrome, Internet Explorer, and Firefox.
Some of my information is wrong. How do I correct this?
If you notice an error in your personal information, such as address and
phone number, please send us a change request through the patient portal.

How do I communicate my medical concerns?
My BMH Health is not used to communicate medical concerns to your doctor.

PHYSICIAN'S OFFICE IMMEDIATELY.
How do I see my test results?
Your test results and reports are both available by clicking on the
Health Record button on the patient portal home page. Once you click on the
Health Record button you will be taken to the Health Record page. You will see buttons
on the right labeled
Reports, which is where you will have access to your Laboratory Results and other reports.
How do I get my complete medical records?
You can obtain your medical records by presenting with appropriate photo
identification such as a driver's license to the Medical Records Department,
located on 1 South at Butler Memorial Hospital and filling out the appropriate
