What is a PHO?
A PHO (Provider Hospital Organization) is a model that enables hospitals
and physicians to work together to accomplish common goals including:
- Develop improved methods of health care delivery
- Oversee integration of physicians and hospitals into health care delivery networks
- Assisting in voluntary group formation
- Collect, analyze and disseminate information
The Butler Health System PHO consists of advisory committees and is governed
by a seven person Board of Managers with five being participant physicians.
Care Navigator Program
Recently, the Butler Health System PHO created a Care Navigator program.
This program is designed to help improve the overall health and well-being
of an individual by providing a member focused, team-based approach to

Bridges Health Partners
Bridges Health Partners is a partnership among four independent non-profit
health systems that began in 2015 in the Pittsburgh metropolitan market
and surrounding communities. Consisting of Butler Health System, Excela
Health System, St. Clair Hospital, and Washington Health System, each

Verification of Affiliation
For healthcare facilities who have documented permission to obtain information,
on-line verification is available for provider’s who are actively credentialed with BMH
and who are in good standing. Please note that to verify historical, inactive
providers you may email your request and release to
medicalstaffoffice@butlerhealthsystem.org and we will provide a timely response.
