100 Top Hospitals Award by IBM Watson Health - For a third year, Butler Health System was named one of the nation’s
100 Top Hospitals by IBM Watson HealthTM. Formerly known as the Truven Health Analytics® 100 Top Hospitals,
this study spotlights the top-performing hospitals in the U.S. list based
on a balanced scorecard of publicly available clinical, operational and
patient satisfaction metrics and data.

100 Top Hospitals Everest Award by IBM Watson Health - As an Everest award winner, Butler Memorial Hospital is one of only 13
hospitals in the nation that has simultaneously set the national benchmark
for both balanced excellence in a single year, as well as greatest improvement
over five consecutive years.

Primary Stroke Certification - HFAP is a nationally recognized not-for-profit accreditation organization
with deeming authority from the Centers for Medicare and Medicaid Services.
This designation recognizes the hospital’s commitment to excellence
in the care, stabilization and treatment of stroke patients.

Gold Plus and Target: Stroke Honor Roll - We’re proud to be recognized by the American Heart Association
and American Stroke Association for our active involvement in Get With
The Guidelines®-Stroke, and Target: StrokeSM, a program designed to
help hospitals make sure patients get the benefits of the latest scientific
guidelines-based treatment.

PA Department of Health Stroke Center Designation - The Stroke Center designation from the PA DOH promotes a culture of excellence
at the hospital and gives the community we serve confidence in the quality
of stroke care received at our facility.

Lung Cancer Screening Center Designation - Butler Health System has been designated a Lung Cancer Screening Center
by the American College of Radiology.

Chest Pain Center Accreditation - Butler Health System has been named a Chest Pain Center with PCI (Percutaneous
Coronary Intervention) from the Society of Cardiovascular Patient Care
(SCPC). This program has received a three-year accreditation and is subject
to an ongoing process of performance improvement.

BHS and Wellbe Awarded HHS Grant for Diabetes Self Management - The United States Department of Health and Human Services has awarded
a $200,000 grant to Butler Health System and Madison, WI based-Wellbe
as part of the Rural Health Care Services Outreach Grant Program to support
diabetes self-management using online patient engagement. The project
will serve rural areas of Butler, Clarion, Lawrence, and Venango counties
with an innovative diabetes support and education system coordinated by
Butler Health System and in partnership with Clarion Hospital and Seneca
Medical Center.

American Heart Association Stroke Gold Plus Quality Achievement Award - Butler Health System has received the American Heart Association/American
Stroke Association's Get With The Guidelines Stroke Plus Quality Achievement
Award with Target Stroke Honor Roll Elite. This award recognized the hospital's
commitment and success ensuring that stroke patients receive the most
appropriate treatment according to nationally recognized, research-based
guidelines based on the latest scientific evidence. To receive this award,
hospitals must achieve 85% or higher adherence to all Get With The Guidelines
Stroke achievement indicators for two or more consecutive 12 month periods
and achieved 75% or higher compliance with 5 of 8 Get With The Guidelines
Stroke Quality measures.

2014 APEX Award - Butler Health System has received the Gold Level 2014 VHA Mid-Atlantic
APEX (Achieving Patient Care Excellence) Award. The APEX award honors
hospitals that demonstrate extraordinary levels of patient safety and
clinical quality. BHS was recognized for: Decreasing Central Line Associated
Blood Stream Infection events to zero for a consecutive 12-month period
from July 1, 2013 to June 30, 2014; decreasing inpatient Ventilator Associated
Pneumonia to zero in critical care units for a consecutive 12-month period
from July 1, 2013 to June 30, 2014; and decreasing inpatient Venous Thromboembolism
events to a rate of < or = to 0.6 for at least a consecutive 6-month
period from July 1, 2013 to June 30, 2014.

2014 CI+T Accelerator Awards, hosted by Clinical Innovation + Technology
Magazine (September 2014) - This program recognized innovative projects that are helping healthcare
providers achieve the triple aim. Butler Health System’s use of
Wellbe’s Guided CarePaths for total joint replacement and bariatric
surgery patients won in the Patient Engagement Category. There were a
total of five winners across all categories for 2014.

Women’s Choice Award, Emergency and Orthopedic Care, 2014 & 2015 - Butler Health System has been named a recipient of the Women’s
Choice Award as one of America’s Best Hospitals in Emergency and
Orthopedic care for the second year in a row. The award is based on robust
criteria that consider patient satisfaction, clinical excellence and what
women really want when it comes to treatment and a quality hospital experience.

HealthGrades - Cardiac Surgery Excellence Award™ – May 2014 - This award recognizes hospitals for superior outcomes in heart bypass
and heart valve surgery.

American Heart Association, Get With the Guidelines Gold Plus Recognition - Butler Health System received the American Heart Association, Get With
the Guidelines Stroke Gold Plus recognition in June 2014 for maintaining
guidelines set by the American Heart Association for care of stroke patients
for an additional year.

Excellence in Health Care Compliance (Deficiency Free Survey) - Butler Memorial Hospital is the only hospital in Pennsylvania to receive
an “Excellence in Health Care Compliance” award two years
in a row (October 2012 and 2013) from the PA Department of Health. Among
Pennsylvania’s 220 hospitals, Butler and six others received the
award. To be recognized, facilities must be defect-free on the licensure
survey and have no major safety code deficiencies on building inspections.

American Heart Association, Get With The Guidelines Stroke Gold Plus recognition, June 2013 - Recognized for meeting guidelines set by the American Heart Association
for care of Stroke patients for more than two consecutive years.

American Heart Association, Get With The Guidelines Heart Failure - American Heart Association Bronze recognition – Get With The Guidelines
Heart Failure – May 2013, recognized for meeting guidelines set
by the American Heart Association for care of Heart Failure patients for
more than 90 consecutive days.

Primary Care Resource Center - April 2013, one of six community hospitals chosen to participate in
Pittsburgh Regional Health Initiative’s (PRHI) program to create
hospital-based resource centers with the goal of lowering 30-day readmission
rates for chronic obstructive lung disease (COPD), congestive heart failure
(CHF), and coronary artery disease (CAD) by 40% over the next 2.5 years.

Awarded an “A” for Patient Safety - Butler Health System was awarded an “A” for patient safety
by Hospital Safety Score(SM) (Leapfrog Group); recognized for two consecutive
quarters in June 2012 and December 2012.

Honored by PA Department of Agriculture, July 2012 - Butler Health System was recognized for efforts in using local and sustainable
vendors, especially those participating in the PA Preferred Buy Fresh,
Buy Local program.

VHA PA - APEX (Achieving Patient Care Excellence) Award, January 2012 - Butler Health System was recognized for scoring in the top quartile
percentile for the treatment of Acute Myocardial Infarction, Heart Failure,
Pneumonia, and Surgical Care Improvement.

PA Department of Health Stroke Program Certification, August 2012 - Butler Health System is the only Certified Primary Stroke Center between
Pittsburgh and Erie.
