Successful Diabetes Management Begins With EducationThe Diabetes Management Center at Butler Memorial Hospital Can Help Patients With Diabetes Live Better Lives.
Of the estimated 18.2 million adults in the United States who have diabetes, some may not even know they have the disease. Others may be aware they have it but aren't sure how to effectively manage it.
At Butler Memorial Hospital, we've recently developed the Diabetes Management Center to teach those with type 1 and type 2 diabetes how to better manage their disease, including better managing their diet.
We take our patients through a continuum of care, offering them hope and helping them understand how they can enjoy a good quality of life - even with diabetes - through proper managment.
Education: The Key to Managing Diabetes
Knowledge is key to effective diabetes control. You can manage your disease by making educated choices in your daily life. Our classes can teach you how to make these decisions.
View our list of class descriptions.
Continuum of Care
Patients typically come to our Diabetes Management Center on their own, after a referral from their primary care physician, or after having been diagnosed with diabetes as an inpatient.
First-time patients can expect a consultation with a nurse diabetes educator. Our nurse diabetes educators will also outline the Diabetes Education class. Patients may participate in the class in a group setting, one-on-one sessions, or sessions that include them and their families.
Keeping Patients and Primary Care Physicians Engaged
Our Diabetes Management Center is committed to keeping patients and their primary care physicians engaged in the diabetes management process, even after completion of our classes. Patients are sent reminder letters when it comes time for an appointment or exam. They and their primary care physicians are also sent follow-up letters six months after completing the Diabetes Education and Nutrition Therapy classes.
Cost
There is a fee for the classes, but many insurance companies and Medicare cover the cost. Contact your insurer to see whether the classes are covered. If not, please contact the Diabetes Management Center to discuss other options that may be available.
