Diabetes Education
For diabetes patients, caring for the disease is a continuum of education, monitoring and consultation. The first step is a consultation with a nurse diabetes educator, who will outline the available diabetes education classes.
Patients will also learn about the Continuous Glucose Monitoring System (CGMS), a tool for understanding a patient’s personal patterns of glucose control. CGMS is a wearable device which records 72 hours of continuous glucose readings, helping patients identify glucose fluctuations related to food choices, exercise, medications and stress. It will help to identify hypoglycemia, hyperglycemia, the dawn phenomenon and aids in post meal glucose control. There are requirements for wearing the CGMS that will be discussed with you by the Diabetes Management team.
